About the Role

The Foster Care Liaison / Care Coordinator plays a vital role in supporting youth and families involved in the foster care system by ensuring they are connected to the right services at the right time. This position serves as a bridge between The Center and external partners, helping coordinate care, facilitate communication, and maintain continuity through transitions. With a focus on collaboration, organization, and compassion, this role helps guide individuals through complex systems while supporting positive outcomes and long-term stability.

Key Responsibilities:

  • Assist clients through the intake and admission process, including completing paperwork, assessments, and gathering required data

  • Provide ongoing care coordination services aligned with each client’s individualized treatment plan

  • Coordinate services for youth involved in foster care, including engagement, ongoing support, and transition planning

  • Serve as a liaison between The Center and external partners, including DCF, juvenile justice, foster care contractors, and placement providers

  • Facilitate communication and collaboration across healthcare providers, social services, and community resources

  • Verify and document custody status and ensure accuracy of related records

  • Coordinate transitions of care related to placement changes or system involvement, ensuring continuity and follow-up

  • Complete timely and accurate documentation, including assessments, progress notes, and outcomes data

  • Participate in treatment team meetings and agency trainings

  • Support additional duties as assigned

Requirements

Minimum Qualifications
  • Bachelor’s degree in psychology, social work, or a related field or four (4) years of equivalent work experience

  • Valid Kansas driver’s license with a favorable driving record

  • Background free of issues related to abuse or neglect

  • Knowledge of child welfare systems (DCF, foster care, juvenile justice)

Preferred Qualifications
  • Minimum of one (1) year experience working with children, adolescents, or families in behavioral health, social services, or foster care

  • Working knowledge of community and social service resources

  • Understanding of recovery-oriented care and service delivery

  • Experience with program development or service coordination
